Responsibilities Provide case management and prenatal education to individuals and groups of prenatal patients receiving services at Vista Community Clinic.
Rotate through all prenatal service locations as needed.
Utilize OB triage protocol to conduct triage on patients who call or show at the clinic and schedule patients for appointments as appropriate Conduct patient checkout to ensure all requires services were received and that all recommended follow-up appointments and referrals are completed.
Prepare package and complete intake on new patients following established CPSP and VCC protocols Complete and maintain assessments for health education, nutrition and psychosocial CPSP components Provide case management services to clients including coordination with client, family members, client support systems and members of the interdisciplinary team Process necessary referrals and coordinate necessary appointments Qualifications Minimum High school graduate or equivalent Minimum two year's clinical experience (MA) and experience in perinatal care with basic knowledge of Prenatal care for women who are pregnant (OBGYN) CPR Certification (BLS Child, Infant, Adult) Bilingual English/Spanish Valid California driver's license, current vehicle insurance, and reliable transportation to travel between sites for meetings and/or to gather information for reporting purposes; minimum two years' driving experience; with no major violations on driving record Preferred Medical Assistant certificate Minimum three years' maternal and child health experience, including direct patient services CPHW Certificate Required Skills/Knowledge/Abilities Experience with a Medi-Cal program in an outpatient setting Basic computer knowledge, internet use, MS Office and data entry skills Job Type:
