Summary:
Under the supervision of the Facility Manager the Educational Liaison/Care Coordinator is responsible for anything related to the coordination of care for all ROWI clients, including developing an education plan for all ROWI PHP clients.
The Educational Liaison will coordinate educational plans with the clients home school and parents in order to best meet the needs of the client.
The Educational Liaison/Care Coordinator will coordinate individualized service including orientation, transportation, doctor visits, and schedule.
Duties and
Responsibilities:
Oversee the development and day to day management of the classroom.
Communicate with the home school and provide updates as needed.
Complete educational assessment with all clients within 3 days of admission & upload to KIPU chart.
Participate in IEP meetings as needed.
Document all classroom activity in KIPU.
Ensures programs and services are implemented at the highest standards and clients receive the highest level of care.
Facilitate assigned groups and activities as designated by the Program Director.
Conduct client orientation for all admits on day of admission or assign to another staff if not available.
Including UA screening, PHQ9 and other assessments as directed.
Ensure compliance with Joint Commission Standards.
Document shift reports and share pertinent information with the team.
Coordinate the transportation schedule for all clients and drive clients as needed.
Document all transportation and group activity in KIPU.
Administer and submit documentation in KIPU for client Urine Analysis.
Assist with facility cleanliness as needed.
Serve as a role model to treatment staff, clients, employees, and guests through a positive, caring, and professional attitude.
Maintain the confidential nature of clients and related activities.
Comply with the dress code.
Complete tasks in a timely and accurate manner.
Attend designated staff and in-service meetings.
Any additional duties as assigned by the management team.
Ability to maintain clear boundaries with clients and staff, set limits and manage clients when in need of direct supervision.
Maintains appropriate and professional interactions with clients and staff.
All documentation in KIPU must be submitted within 24 hours.

Basic Education and Experience Required:
The Educational Liaison will have a minimum of a high school diploma, Bachelor's degree preferred.
Minimum of two (2) years driving experience required.
Minimum of 1-year working with kids in a therapeutic setting Experience with junior high or high school students is preferred.

Additional Requirements:
Must have a valid driver's license and eligibility for insurance with agency's vehicles.
Must have valid First-Aid & CPR Certifications and a criminal background check.
