Job Description

A Vista, California based client is seeking a motivated and compassionate Nurse Practitioner to join a PACE team.
An ideal candidate should practice excellent, evidence-based patient care, and should be comfortable treating older adults in a PACE facility.
Managed care experience and community health experience are a plus.

Required Qualifications

  • CA NP license
  • CA DEA certificate before start date
  • CPR certification
  • Board certified in field

Responsibilities

  • Assess and diagnose a variety of diseases and injuries in older adults
  • Order diagnostic tests and laboratory studies, interpret results and create treatment plans based on results
  • Perform therapeutic procedures with prioritizing preventative care
  • Arrange referrals, consultations, and therapeutic services for patients on caseload
  • Document all patient encounters efficiently and according to organization policy
